Birth certificates prior to 1990 cost $15.00 and 1990 to the present cost $10.00 (certified computer copies).

Personal check or money order should be made payable to Office of Vital Records. To verify current fees, the telephone number is (602) 364-1300. This will be a recorded message. Information on how to obtain certified copies is also available via the Internet at: http://www.hs.state.az.us/

Applicants must submit a copy of picture identification or have their request notarized.
